Intensive care at home is the health service we provide to our patients who are in acute, chronic or terminal period with our health team in their living areas. This service, which is given in short or long term according to the needs of our patients, is done through home care doctors and executive nurses on the patients,
- Who have a risk of death and are requiring permanent medication,
- Whose treatment will be continued at home,
- Who have bedsores,
- Who are connected to with respiratory device,
- Who are cancer in terminal period,
- Who will be discharged from the intensive care unit to home.
The process for our patients or patients’ relatives who are seeking intensive care nurse services at home is as follows;
- On behalf of the patient to be seen and evaluated, the home care nurse visits the site.
- In order for the patient to benefit from intensive care at home, environmental conditions are evaluated and any deficiencies are eliminated.
- Installation of medical devices is provided if necessary.
- The home care nurse carries out the filling of the forms in order to meet the patient’s needs and to evaluate the patient’s condition according to the treatment planned by the attending physician in this field. Regularly informs the doctor and family relatives about the patient’s health status.
